Delta dental plans

PPO Plan

The PPO plan gives you access to the largest network of dentists in the State of Illinois and the option of seeing a Delta Dental provider anywhere in the country. The plan features benefits for services both in and out-of-network. If you visit an in-network dentist, you will receive a larger annual maximum benefit. Highlights of the PPO plan include: 

  • No waiting periods.
  • Preventive and Diagnostic services are covered at 100%.  
  • Additional Preventive visits provided if you qualify for the Enhanced Benefits Program.
  • Ability to select the dentist of your choice.
  • Annual maximum benefit of $1,500 per-person with in-network dentists.

How to find a participating PPO Dentist near you:

  1. Click here for the Delta Dental provider search.
  2. To start your search, you can either enter the location where you want to locate network dentists (search by city/state or ZIP code), or search for a particular dentist or practice by name and ZIP code. Use the “Select your Network” field to choose either the Delta Dental PPO or Delta Dental Premier network.
  3. You can further narrow your search by selecting a specialty (such as orthodontist), languages spoken and gender. Additional filters, like languages spoken and distance, can be used to narrow your search. Please note: the PPO Plus Premier Plan allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PPO network.

DeltaCare DHMO Plan

The DeltaCare DHMO plan is a great option for students seeking dental coverage with lower costs. Enrolled students select from a panel of primary care dentists, and enjoy minimal co-payments for basic and major services. You may change your dentist if you wish, but you must select another dentist from the DHMO network. Specialty care services will require a referral from your primary care dentist. Highlights of the DeltaCare DHMO include:

  • Low premiums.
  • No waiting periods.
  • Preventive and Diagnostic services are covered at 100%.
  • No deductibles or annual maximums.
  • Easy to understand copayment schedule with fixed costs for services.
  • Coverage for orthodontic services.

Please note: You must select a Primary Care Dentist in Illinois to participate in the DeltaCare DHMO plan.

How to find a participating DeltaCare DHMO Dentist near you:

  1. Click here for the Delta Dental provider search.
  2. Start your search by entering the location where you want to locate network dentists (search by city/state or ZIP code). Use the “Select your Network” field to choose the DeltaCare network.
  3. The facility ID code for the dental provider you choose can be found under the practice name and address. This facility ID must be entered during enrollment. Please note: Facility ID’s must be provided by the 15th of the month to appear on the facility roster for the following month. DeltaCare DHMO plan members must select a general dentist in the Illinois DeltaCare network.

Deltavision plan

The DeltaVision plan uses the EyeMed Select network and includes in-network access to local providers, popular retail chains, and online ordering options through Contacts Direct and Glasses.com. The plan provides coverage for both in and out-of-network eye care providers. You will maximize your benefit by using in-network providers. Highlights of the plan include:

  • No waiting periods.
  • A routine eye exam for a $10 copay.
  • Contact lenses for a $0 copay
  • Standard prescription lenses for a $25 copay and a $100 frame allowance.
  • Discounts on LASIK and PRK.
  • Up to 40% off additional vision care purchases.

How to Find a Provider in the EyeMed Select Network

  1. Click here to view the EyeMed Vision Care provider directory.
  2. Enter your city or zip code, and choose the “Select” network to generate a list of vision care providers near you.
  3. You can filter your search results by hours and scheduling, services, products, and brands.

How to Use Your Vision Benefits

  • An ID card is not needed to use your vision benefits. Your EyeMed provider just needs your name and date of birth to verify coverage at the time of service. You can print this DeltaVision Card to have your plan information handy whenever you visit your vision care provider.

    If you visit an in-network provider:

    • You will pay your co-payment and any other charges not covered at the time of service.

    If you visit an out-of-network provider:

NU-Supplement

Students may now enroll in an Accident and Sickness Hospital Indemnity Insurance plan called the NU-Supplement. You will receive cash benefits when you suffer certain accidental injuries, are confined to a hospital, or experience certain unexpected expenses related to a hospitalization. The supplement is provided to help you cover the out-of-pocket expenses you may incur like your deductible and co-pays. This plan pays claims resulting from both accidents and sickness. You are not required to use the cash payments to pay the provider. The benefits are paid directly to you, and you may use the claim payment however you choose. Your coverage will become effective the first of the month, following the date of enrollment.

  • No deductible
  • Cash benefits paid directly to you, even if you have other insurance
  • Hospital admission benefit $1,000
  • In-hospital benefit $250 per day
  • Intensive Care Unit benefit $500 per day
  • Emergency Room $250 per visit
  • Surgical benefit $1,000 to $2,500
  • Fracture benefit up to $1,000
  • Ambulance transportation benefit $500 – $2,000

How to Submit a Claim

You may submit a claim form by mail, email, or fax.

  1. Answer each item on page 1 and sign the form where indicated. Your Member ID is your Student ID number.
  2. Include itemized medical bills and your Explanation of Benefits (EOB).
  3. A claim form should be submitted within 20 days of the occurrence.
